Falls are one of the leading causes of injury among older adults, but many falls can be prevented with early awareness and the right support. Our quick online Fall Risk Assessment is designed to help seniors, caregivers, and family members better understand common warning signs that may increase the risk of falling at home or during daily activities.

This easy self-screening tool asks a series of simple questions related to balance, walking, medications, mobility, and overall confidence while moving around. Based on your answers, you will receive a general fall risk evaluation along with practical recommendations that may help improve safety and reduce the chance of injury.

Many people do not realize they are at increased risk until after a fall has already occurred.
